For Patients — Evolent Surgical Management

We partner with your health insurance plan and facility to provide the medical device, biologics or other tools used in your surgery.

How the Surgical Management Program Works

  • We fund the implant or medical device used in your surgery.
  • We bill your health insurance plan and obtain reimbursement for the device, tools or biologics used in your surgery.
  • We send a statement outlining your financial responsibility based on your insurance policy and benefits.
  • The goal? Lower out-of-pocket expenses for you and your loved ones.

 

Your Responsibility

You are responsible for paying the outstanding balance to Evolent for the implant device portion of your procedure, which may include any unmet deductible, co-insurance or other out-of-pocket expenses.

 

Consent Form

To ensure we have the information we need to properly bill your health insurance plan, you may be asked to complete an Evolent consent form at your facility. Click here to view a copy of our consent form.
